Our Alternative Living Services (ALS) – Family Model program provides a safe, supportive, and comfortable residential environment for individuals who need assistance with daily living while remaining connected to their community.
Through Georgia’s Elderly and Disabled Waiver Program (EDWP), eligible individuals may receive Alternative Living Services as an alternative to nursing facility placement.

Our leadership has over 3 decades of experience in ALS in Georgia, more than any competitor! Indeed, Mr. Petrea was a pioneer in ALS Services in the early days of the waiver. We have owned and operated Personal Care Homes and supervised Family Model Homes for decades. We understand the service like you do.
A FAMILY STYLE HOME
We believe a care home should feel like a home—not an institution.
ALS–Family Model services are provided in a state-licensed personal care home serving 2 to 6 residents. This smaller setting allows individuals to receive individualized support while enjoying the comfort and familiarity of a home environment.
Residents are encouraged to participate in everyday activities, make choices, develop relationships, and remain connected to their families and communities.

Managed by Longleaf Residential
Longleaf Residential serves as the management company for our ALS–Family Model homes.
Longleaf Residential provides administrative and operational management to support the successful operation of each home. Our management team works closely with home providers, caregivers, nurses, families, care coordinators, and other members of the individual’s support team.
Our focus is to ensure each home has the resources, guidance, oversight, and support necessary to provide a safe, comfortable, and person-centered living environment.
Our Management Responsibilities
Our Management Responsibilities Include:
- Administrative oversight of the ALS homes
- Support with regulatory and program compliance
- Routine nursing supervisory visits
- Coordination with home providers
- The provision of training to ensure compliance
- Oversight of required documentation
- Quality assurance and monitoring
- Support for health and safety requirements
- Coordination with nursing and healthcare professionals
- Communication with families and representatives
- Support with care coordination
- Ensuring accurate and timely payment
